Treating Rural America – Multispecialty in Home

Mr. Thomas Hutto lives on his own in Garrison, Texas (population 846). Mr. Hutto has advanced COPD, congestive heart failure, and CKD. Before Monogram, Mr. Hutto had to travel 2+ hours to see a specialist or visit the nearest “walk in only” Urgent Care for any exacerbations he was experiencing or to get his medications refilled/reviewed. Since Monogram, Mr. Hutto has been able to stay in his home and receive the multispecialty treatment and care he needs on his time. He now has direct access to clinical tests, pharmacists, nurses, doctors, and specialists 24/7 at his kitchen table.
