Understanding Monogram

Monogram Health is a team of healthcare experts who help people manage multiple health conditions from the comfort of their home. A care team of doctors, nurse practitioners, specialists, nurses, pharmacists, social workers, and others help with all of your health needs —not just one at a time.

We can visit you at home, talk with you by phone, or meet by video. We also work with the doctors you already know and trust to help keep your care connected.

We’re here to support you in the way that fits your life.

You may have been contacted because your health plan wants you to know you have extra support managing your health. Monogram works with many health plans to help people living with ongoing health problems get the care, education, and support they need.

Our goal is to help you stay healthier, avoid extra hospital visits, and make managing your health a little easier.

Your doctor usually sees you during scheduled visits or when you’re sick. Monogram offers extra support between those visits – and for as much time as you need. We help answer questions anytime you have them (day or night and on holidays), check in on how you’re doing, and support your care plan over time. Think of us as an added layer of support, working with your doctor to help you stay as healthy as possible.

Monogram works with many health plans to provide care services to members who qualify. Monogram is not an insurance company but partners with participating health plans.

Whether you qualify depends on your health plan and health needs. Monogram often supports people living with chronic or complex health conditions, but qualification varies by health plan.

No. Participation is always your choice. We encourage you to learn about the program, ask questions, and decide whether Monogram is right for you.

Participating in Monogram’s Care Management Program is free. There is no charge for any care support you receive from Monogram – like help from a Monogram RN, pharmacist, or social worker. 

However, if you choose to receive covered medical services – such as a medical exam, lab tests, or treatment from a Monogram nurse practitioner, doctor, specialist, or other provider, those services may have cost for you.

Monogram’s care providers are part of your health plan’s provider network, so your plan’s standard benefits and cost-sharing rules apply to any medical services they provide. This means you could have a deductible, copay, or coinsurance applied by your health plan, just as you would with any other in-network provider visit

Some visits with doctors, nurse practitioners, or specialists may have a co-pay, coinsurance, or deductible depending on your health plan. To find out what’s covered and if any costs apply, give your health plan’s customer service team a call.

Getting started is easy. Call Monogram Health at (855) 451-3293 and one of our team members will answer your questions, explain the program, and help you determine whether Monogram is right for you. If you choose to participate, we’ll help schedule your first visit and begin building a care plan tailored to your needs.
We’ll begin by learning about your health, medications, health goals, and the care you currently receive. Together, we’ll create a care plan made just for you and connect you with the members of your care team who can best support your needs.
Participation in Monogram is always your choice. If you would like to stop receiving services, simply call (855) 451-3293. An Engagement Specialist will assist you with the process and answer any questions you may have.

At Monogram Health, protecting your health information is very important to us. We follow federal and state privacy laws, including HIPAA, and use secure technology to help keep your information safe.

Your Care Team

Your Monogram care team is made up of healthcare professionals who work together to support your health. Depending on your needs, your team may include nurse practitioners, nurses, doctors, specialists, pharmacists, social workers, dietitians, and behavioral health experts. We work closely with you and the doctors you already trust to help keep your care connected.

Yes! You can keep your current doctors! Monogram works alongside your healthcare providers to give you additional support between visits, and answer questions when you need help. Monogram is also here when your doctor’s office is closed—like at night, on weekends, or during holidays.

Monogram provides access to a range of specialists who help manage complex and chronic health conditions. Specialty support may include:

  • Kidney care (Nephrology)
  • Heart care (Cardiology)
  • Lung care (Pulmonology)
  • Diabetes care (Endocrinology)
  • Comfort and symptom support (Palliative care)
  • Support for stress and life challenges (Behavioral health)
  • Help with everyday needs (Social work)
  • Medication support (Pharmacy)
  • Healthy eating support (Nutrition and dietitian services)

Your care team connects you with the right specialists based on your individual health needs.

Monogram nurse practitioners and specialists can review your medications, prescribe medications when appropriate, and help make sure your treatments are working well together. We also work closely with your other healthcare providers to keep everyone informed about your care.

Appointment & Support

During your visit, we’ll talk about your health and what matters to you. We’ll review your health history, including any medicines you take and any recent surgeries or procedures. We’ll also check important things like your vitals. We’ll ask some questions, but this visit is about you. If something is on your mind, we want to hear about it.

We work with your schedule and preferences. Visits can take place in your home, by phone, or by video.

There’s always time to talk about your questions or concerns.

Some questions may sound familiar because your doctor has asked them before. We do this on purpose. Asking again helps us confirm information, identify changes in your health, and make sure your care plan stays up to date. Even if it feels repetitive, it helps us provide the best possible care.
Monogram appointments can take place in your home, by phone, or by video, depending on your needs and preferences. We work with you to find the type of visit that is most convenient and comfortable.
The frequency of visits depends on your health needs and goals. Some patients benefit from more frequent visits, while others may only need periodic check-ins. Your care team will work with you to create a schedule that fits your needs.

You can call Monogram Health FIRST 24 hours a day, 7 days a week. A real person always answers the phone.

Services & Benefits

Monogram helps people manage chronic and complex health conditions such as kidney disease, heart disease, diabetes, COPD, and other ongoing health concerns. We focus on your overall health and help connect care across all your conditions.

Yes. Many Monogram patients live with more than one health condition. Instead of treating each condition separately, we help connect all parts of your care so your treatments, medications, and healthcare providers work together.

Your health is affected by more than medical care alone. Monogram’s social workers can help connect you with local programs and services that may support:

  • Transportation to medical appointments
  • Access to food and nutrition programs
  • Housing-related resources
  • Financial assistance programs
  • Medication access support
  • Feeling alone or needing extra support

While Monogram does not directly provide these services, we help connect you to resources available through your community, health plan, or local organizations.

You can reach out anytime you have a question or need extra support. You’re never on your own.

Yes. Our pharmacy and care team can review your medications, answer questions, help identify potential concerns, and work with your healthcare providers to make sure your treatment plan is safe and working well together.

No, Monogram is not hospice care. We help people manage serious complex and chronic health problems and improve their quality of life. For some patients, Monogram may also help coordinate palliative care to support comfort and symptom relief.

Caregivers

We’re happy to work with you and your caregiver. This can be a family member, a friend, or someone else who helps care for you. They are also welcome to join visits and conversations. Our social workers can even help caregivers find resources and support to make things easier for them.
Yes. They are welcome to join visits, ask questions, and be part of care discussions.
If you have given us permission to share your health information with them, or if they are your personal representative (such as a Healthcare Power of Attorney or healthcare proxy), we can share health information with them.
Yes. Caring for someone with multiple health conditions can be challenging. Monogram helps caregivers understand care plans, coordinate appointments, manage medications, and find community resources that may provide additional support.
