I Took Back Control of My Life Once I Understood the Full Picture

Chip Matthews’s diabetes and blood pressure were out of control and left unmanaged, which led to the decline of his kidney function. Until Monogram Health stepped in, Mr. Matthews and his primary care physician were unaware that Mr. Matthews had kidney disease. Once engaged, the Monogram Health clinical care team was able to review and prescribe medication that would help control his kidney, diabetes, and blood pressure conditions. Monogram Health was also able to help educate Mr. Matthews on his treatment options and assign a dietician to assist with his ongoing care plan.
