Closing Gaps in Care Does Not Have to Be Costly

Wanda Buckner lives on her own and suffers from COPD and chronic kidney disease. Over time, Ms. Buckner was unable to afford her COPD medication and risked having to be hospitalized. One call to Monogram Health First changed everything. A Monogram Health pharmacist, clinician, and social worker collaborated to find a solution to Ms. Buckner’s problem and close the gap in her care.
