Treating Rural America – Multispecialty in Home

Mr. Thomas Hutto lives on his own in Garrison, Texas (population 846). Mr. Hutto has advanced COPD, congestive heart failure, and CKD. Before Monogram, Mr. Hutto had to travel a great distance to see a specialist or visit the nearest “walk in only” Urgent Care for any exacerbations he was experiencing or to get his medications refilled/reviewed.
