Our committed team of doctors, nurses, social workers and more can serve as, or work with, your doctor(s) to provide extra support and personalized treatment right in your home. We design a treatment plan that is unique to you.
With Monogram, members have access to a complete advanced care team that is connected and centered around you. We don’t just treat one condition— we look at your whole health.*For most of our patients, there is no additional costs outside of their regular co-pays.

You can talk to a doctor, nurse practitioner, or nurse anytime, 24/7. We help you right away when you have questions or new symptoms.
You choose how you want care: by phone or right at home. We make it easy to stay on top of your health without extra stress.
When the unexpected happens, like a storm or power outage, we help before, during, and after so you stay safe and have the medicines and supplies you need.
If you have a hard medical choice to make, we help explain your options, prepare questions, and make sure you understand what your doctors recommend.
If you go to the hospital, our team follows your care and helps you when you return home. This helps prevent problems or repeat hospital visits.
If it’s hard to see a local specialist, we can connect you with our experts, like heart doctors, lung doctors, dietitians, or mental health providers.
You can reach a social worker quickly when you need help with resources or support, without waiting for your next visit.
Your care team reviews all your medicines and provides expert help with refills, side effects, and any questions you have.
*Depending on your health plan, a copay may apply to Nurse Practitioner Co-Visits, MD Co-Visits, or Specialist Co-Visits just like the copay you may have when you visit a doctor’s office. If you do have a co-pay, your insurance company will provide you with the cost at the time you process the claim. If the visit only includes a RN, Pharmacist, or Social Worker, there will be no copay for those interactions.
