What to Expect When You Join Monogram Health
Once you’re enrolled in Monogram Health, we schedule a visit where a nurse practitioner conducts a comprehensive assessment of:
- All your medical conditions
- All your medications
- All critical lab work, some of which can be administered right in your home
- Any daily life needs you might need help with, such as food, transportation, financial assistance, or social isolation
Your team of nurses, doctors, social workers and other clinicians will create a treatment plan tailored uniquely to you.
Your dedicated Monogram Health nurse practitioner visits you regularly to provide care, prescribe medications, and help you manage your health, all in the comfort of your home.
Our Monogram Health FIRST Urgent Care at Home service means that medical attention is available to you any time you need it.

Cardiology

Nearly all of our patients have cardiovascular disease, most commonly congestive heart failure and atherosclerotic heart disease.
Through in home care, we help reduce stress on the heart and support better overall health and quality of life.
Pulmonology
PULMONOLOGY
Pulmonary conditions often accompany other chronic conditions. In home diagnostic tools, like spirometry, allow for early diagnosis, care, and treatment. Our patients most commonly suffer from Asthma and COPD.
Our team focuses on early symptom relief and uses a whole-person approach to help patients breathe easier and feel better in daily life.
Endocrinology
ENDOCRINOLOGY
Many of our patients have diabetes which can increase disease progression across the blood vessels, heart, and kidney.
Our in-home care connects with all other specialties to help patients keep their conditions under control and avoid complications.
Nephrology
NEPHROLOGY
Kidney disease is a common complication of patients with other chronic metabolic conditions. Many of our patients have kidney function less than 45%. Of those patients with progressed kidney disease, approximately 10% have End Stage Kidney Disease (ESKD).
We have numerous clinical interventions available to help slow the progression of kidney disease and improve the quality of life of patients.
At Monogram, we look at Advanced Kidney Clinical Pathways including conservative kidney management, multidisciplinary care, symptom management, and patient-centered decision making.

Palliative Care
PALLIATIVE CARE
About 30 percent of our patients are candidates for palliative care. Palliative care is part of every specialty at Monogram.
When used early, it can greatly improve comfort, reduce symptoms, and support better overall health.
Behavioral Health
BEHAVIORAL HEALTH
About 30% of Monogram patients have mental or behavioral health needs. That’s why we include these services in every home visit.
We use face-to-face conversations and simple screening tools to understand what each patient needs.
Our Chief Psychiatrist helps guide the care plans and treatments we offer.

Social support is often overlooked as being integral to patient success. At Monogram health, early engagement and recognition of social needs allow for better whole-person care.
We know that things like housing, food, finances, and transportation affect health. We provide this extra support and help with these needs.
