Finally, My Healthcare Provider Sees Me and Is Speaking My Language

Dennis Eleuteri lives on his own and suffers from uncontrolled diabetes, kidney disease, congestive heart failure and a severely wounded foot (as a result of his diabetes). Additionally, Mr. Eleuteri faces a myriad of social barriers, such as: illiteracy, living costs, affordable housing, access to food, transportation, and financial management planning. Before Monogram Health, Dennis was routinely being admitted to the hospital and/or a skilled nursing facility. Monogram Health’s robust team connected and were able to treat all of Mr. Eleurteri’s clinical and non-clinical needs in the home. Addressing Mr. Eleuteri’s social barriers with a Monogram Health social worker directly improved Mr. Eleuteri’s medical outcomes too.
