Mary Hawkins lives alone and was experiencing symptoms of a UTI infection. After one call to Monogram, she received both the clinical and non-clinical care/treatment she needed. In addition to her urgent and nonurgent medical needs, Monogram became aware that Ms. Hawkins was in fear of losing her home because the deed was in her late grandmother’s name. Monogram Health was able to step to source and secure the legal guidance and documentation needed to transfer the paperwork to Ms. Hawkins.
